The Family Care Connections workgroup designs Family Care Connections that prioritize family and youth voice and choice in the development and implementation of an individualized and coordinated plan of care that includes pediatric primary care, schools, and behavioral health in order to support and enhance the social, emotional, and physical health of children, youth, and their families.
